    A randomized controlled trial to evaluate the effect of a new skincare regimen on skin barrier function in those with podoconiosis in Ethiopia.

    BACKGROUND: Podoconiosis affects an estimated 3 million people in Ethiopia with a further 19 million at risk. Volcanic soil and pathogens enter skin breaches in the feet causing inflammation, lymphoedema and hyperkeratosis. There is no robust evidence on optimal podoconiosis skincare regimens to improve skin barrier function (SBF). OBJECTIVES: To evaluate the effectiveness of a new, low-cost, evidence-based intervention to improve SBF in the lower limbs of those with podoconiosis. METHODS: A randomized controlled trial (NCT02839772) was conducted over 3 months in two podoconiosis clinics (n = 193). The intervention comprised 2% (v/v) glycerine added to a reduced volume of soaking water. The control group received the current skincare regimen. Primary outcome measures were transepidermal water loss (TEWL) and stratum corneum hydration (SCH) at four specific sites on the lower limbs. RESULTS: Improvement in SBF was observed in both groups across all measurement sites and time points, although this was significantly greater in the experimental group. TEWL reduced in both groups at all sites. For example, on top of the foot the estimated group difference in TEWL at visit 4 was 1·751 [standard error (SE) = 0·0390] in favour of the experimental group [t = 3·15, degrees of freedom (df) = 189·58, P = 0·002, 95% confidence interval (CI) 0·066-2·85], indicating a greater reduction in TEWL in the experimental group. Similarly, at the same site the estimated group difference in SCH at visit 4 was -2·041 (SE = 0·572) in favour of the experimental group (t = -3·56, df = 186·74, P < 0·001, 95% CI -3·16 to -0·91), indicating a greater increase in SCH in the experimental group. There were also significantly greater reductions in odour, number of wounds and largest foot circumference in the experimental vs. the control group. CONCLUSIONS: The addition of 2% (v/v) glycerol to a reduced volume (83% reduction) of soaking water significantly improved SBF

    We report the design and testing of a compression-biased thermally-actuated deformable mirror that has a dynamic range larger than the limit imposed by pure-bending stress, negligible higher-order-mode scattering, and a linear defocus response and that is vacuum compatible.     The effect of multiple deformations on the formation of ultrafine grained steels

    A C-Mn-Nb-Ti steel was deformed by hot torsion to study ultrafine ferrite formation through dynamic strain-induced transformation (DSIT) in conjunction with air cooling. A systematic study was carried out first to evaluate the effect of deformation temperature and prior austenite grain size on the critical strain for ultrafine ferrite formation (&epsilon; C,UFF) through single-pass deformation. Then, multiple deformations in the nonrecrystallization region were used to study the effect of thermomechanical parameters (i.e., strain, deformation temperature, etc.) on &epsilon; C,UFF. The multiple deformations in the nonrecrystallization region significantly reduced &epsilon; C,UFF, although the total equivalent strain for a given thermomechanical condition was higher than that required in single-pass deformation. The current study on a Ni-30Fe austenitic model alloy revealed that laminar microband structures were the key intragranular defects in the austenite for nucleation of ferrite during the hot torsion test. The microbands were refined and overall misorientation angle distribution increased with a decrease in the deformation temperature for a given thermomechanical processing condition. For nonisothermal multipass deformation, there was some contribution to the formation of high-angle microband boundaries from strains at higher temperature, although the strains were not completely additive.<br /

    We have searched for the proton decay mode proton to neutrino K+ using the one-kiloton Soudan 2 high resolution calorimeter. Contained events obtained from a 3.56 kiloton-year fiducial exposure through June 1997 are examined for occurrence of a visible K+ track which decays at rest into mu+ nu or pi+ pi0. We found one candidate event consistent with background, yielding a limit, tau/B > 4.3 10^{31} years at 90% CL with no background subtraction.Comment: 13 pages, Latex, 3 tables and 3 figures, Accepted by Physics Letters

    Northwest Pacific ice-rafted debris at 38°N reveals episodic ice-sheet change in late Quaternary Northeast Siberia

    The ice-rafted-debris (IRD) record of the open Northwest Pacific points towards the existence of substantial glacial ice on the Northeast Siberian coast during the late Quaternary. However, the scale and timing of glaciation and de-glaciation remains controversial due to the dearth of both onshore and offshore records. Existing IRD data suggests at least one event of dynamic and abrupt change during mid-late Marine Isotope Stage (MIS 3) which mimics the massive collapse of the Laurentide ice sheet during Heinrich Events. It is uncertain whether other events of this magnitude occurred during the late Quaternary. Here we present a ∼160,000 yr IRD series, planktic foraminiferal counts and an age model, derived from a benthic δ18O curve, radiocarbon dates and tephrochronology, from core ODP 1207A (37.79°N, 162.75°E), revealing the presence of low but episodic flux of IRD. We conclude that glacial Northwest Pacific icebergs spread further south than previously thought, with icebergs emanating from Northeast Siberia being transported to the transition region between the subpolar and subtropical waters, south of the subarctic front during at least the Quaternary's last two glacial periods. The episodic nature of the 1207A IRD record during the last glacial, combined with coupled climate-iceberg modelling, suggests occasional times of much enhanced ice flux from the Kamchatka-Koryak coast, with other potential sources on the Sea of Okhotsk coast. These findings support the hypothesis of a variable but extensive ice mass during the last glacial over Northeast Siberia, particularly early in the last glacial period, behaving independently of North American and Eurasian ice masses. In strong contrast, IRD was absent during much of the penultimate glacial Marine Isotope Stage (MIS) 6 suggesting the possibility of very different Northeast Siberian ice coverage between the last two glacial periods

    Controversies and priorities in amyotrophic lateral sclerosis

    Two decades after the discovery that 20% of familial amyotrophic lateral sclerosis (ALS) cases were linked to mutations in the superoxide dismutase-1 (SOD1) gene, a substantial proportion of the remainder of cases of familial ALS have now been traced to an expansion of the intronic hexanucleotide repeat sequence in C9orf72. This breakthrough provides an opportunity to re-evaluate longstanding concepts regarding the cause and natural history of ALS, coming soon after the pathological unification of ALS with frontotemporal dementia through a shared pathological signature of cytoplasmic inclusions of the ubiquitinated protein TDP-43. However, with profound clinical, prognostic, neuropathological, and now genetic heterogeneity, the concept of ALS as one disease appears increasingly untenable. This background calls for the development of a more sophisticated taxonomy, and an appreciation of ALS as the breakdown of a wider network rather than a discrete vulnerable population of specialised motor neurons. Identification of C9orf72 repeat expansions in patients without a family history of ALS challenges the traditional division between familial and sporadic disease. By contrast, the 90% of apparently sporadic cases and incomplete penetrance of several genes linked to familial cases suggest that at least some forms of ALS arise from the interplay of multiple genes, poorly understood developmental, environmental, and age-related factors, as well as stochastic events

    PTF1 J082340.04+081936.5: A hot subdwarf B star with a low-mass white dwarf companion in an 87-minute orbit

    We present the discovery of the hot subdwarf B star (sdB) binary PTF1 J082340.04+081936.5. The system has an orbital period of P orb = 87.49668(1) minutes (0.060761584(10) days), making it the second-most compact sdB binary known. The light curve shows ellipsoidal variations. Under the assumption that the sdB primary is synchronized with the orbit, we find a mass of M sdB = 0.45 +0.09 -0.07 M ⊙ , a companion white dwarf mass of M WD = 0.46 + 0.12 -0.09 M ⊙ , and a mass ratio of q = M WD /M sdB = 1.03 +0.10 -0.08 . The future evolution was calculated using the MESA stellar evolution code. Adopting a canonical sdB mass of M sdB = 0.47 M ⊙ , we find that the sdB still burns helium at the time it will fill its Roche lobe if the orbital period was less than 106 minutes at the exit from the last common envelope (CE) phase. For longer CE exit periods, the sdB will have stopped burning helium and turned into a C/O white dwarf at the time of contact. Comparing the spectroscopically derived log g and T eff with our MESA models, we find that an sdB model with a hydrogen envelope mass of 5 × 10 -4 M ⊙ matches the measurements at a post-CE age of 94 Myr, corresponding to a post-CE orbital period of 109 minutes, which is close to the limit to start accretion while the sdB is still burning helium

    The Complexity of the Empire Colouring Problem

    We investigate the computational complexity of the empire colouring problem (as defined by Percy Heawood in 1890) for maps containing empires formed by exactly r>1r > 1 countries each. We prove that the problem can be solved in polynomial time using ss colours on maps whose underlying adjacency graph has no induced subgraph of average degree larger than s/rs/r. However, if s3s \geq 3, the problem is NP-hard even if the graph is a forest of paths of arbitrary lengths (for any r2r \geq 2, provided s<2r(2r+1/4+3/2)s < 2r - \sqrt(2r + 1/4+ 3/2). Furthermore we obtain a complete characterization of the problem's complexity for the case when the input graph is a tree, whereas our result for arbitrary planar graphs fall just short of a similar dichotomy. Specifically, we prove that the empire colouring problem is NP-hard for trees, for any r2r \geq 2, if 3s2r13 \leq s \leq 2r-1 (and polynomial time solvable otherwise). For arbitrary planar graphs we prove NP-hardness if s<7s<7 for r=2r=2, and s<6r3s < 6r-3, for r3r \geq 3. The result for planar graphs also proves the NP-hardness of colouring with less than 7 colours graphs of thickness two and less than 6r36r-3 colours graphs of thickness r3r \geq 3.Comment: 23 pages, 12 figure

    Exploring leadership in multi-sectoral partnerships

    This article explores some critical aspects of leadership in the context of multi-sectoral partnerships. It focuses on leadership in practice and asks the question, `How do managers experience and perceive leadership in such partnerships?' The study contributes to the debate on whether leadership in a multi-sectoral partnership context differs from that within a single organization. It is based on the accounts of practising managers working in complex partnerships. The article highlights a number of leadership challenges faced by those working in multi-sectoral partnerships. Partnership practitioners were clear that leadership in partnerships was more complex than in single organizations. However, it was more difficult for them to agree a consensus on the essential nature of leadership in partnership. We suggest that a first-, second- and third-person approach might be a way of better interpreting leadership in the context of partnerships
